Hormone Replacement Therapy: What to expect before and at your first appointment.

Before your first appointment
- Once your appointment is made, a medical history questionnaire will be mailed to you. Please complete this form in its entirety and bring it with you to your first appointment.
- If your insurance carrier requires a referral, you may download our referral form and have completed and faxed to the Center.
- Understand fees and payment expectations by calling toll free 1-866-443-4276.
At your first appointment
- You will see the physician for a 30-minute visit. During this consultation, you will review the medical history questionnaire and a brief physical will be conducted by the physician.
- Extensive laboratory work may be required at this visit*. In this case, additional time may be needed. In preparation, please do not take any thyroid medication the day of your appointment. For a morning appointment, do not eat, but drink plenty of water. For an afternoon appointment, only eat dry toast or crackers with skim milk and drink plenty of water. *In rare cases, lab tests may be delayed on day of appointment.
- Payment is expected at the time services are rendered. We will provide you with a completed billing statement of all charges for submission to your health insurance company.
- Subsequent appointments will be made at that time.
